The Organization

The Ministry of Health and Long Term Care started implementing Family Health Teams all across Ontario in 2005 and the North York Family Health Team was incorporated in June 2007 and became fully operational in 2008.

The North York Family Health Team (NYFHT) is composed of a multidisciplinary team of Registered Nurses, Social Workers, Registered Dietitians, Nurse Practitioners and Pharmacists and over 61 Family Physicians. There are approximately 60,000 patients rostered with the FHT Family Physicians.
