



























































The NYFHT serves the diverse population of North York by providing comprehensive primary healthcare and developing programs focused on health promotion, disease prevention, and chronic disease management through our multidisciplinary team of healthcare providers.
The FHT is composed of Physicians, Registered Nurses, Nurse Practitioners, Registered Dietitians, Social Workers and Pharmacists working together as a team to enhance the care of patients.
Registered Nurse
Our Registered Nurses (RNs) provide professional nursing care and co-ordinate the delivery of medical treatments to patients, ensuring adherence to FHT policies, and nursing procedures, standards and practices. FHT RNs work in close collaboration with FHT Physicians and the other team members to provide more comprehensive care to patients. RNs are accountable for their own professional practice to the College of Nurses of Ontario (CNO).
Nurse Practitioner
The Primary Healthcare Nurse Practitioner (PHC NP) is an advanced practice nurse with knowledge and decision-making skills in assessment, diagnosis, and healthcare management. The NP is able to provide comprehensive health services encompassing health promotion, prevention of diseases and injuries, rehabilitation and support services. The NPs work with the FHT physicians and the other team members to enhance the quality of care and the range of services provided. The PHC NP is a nurse who is registered in the Extended Class (RN EC) with the CNO.
Registered Dietitian
Our Registered Dietitians (RDs) are specifically trained in food and nutrition science in order to help patients achieve their optimal nutritional wellbeing by working with them to plan healthy meal choices for their family, help access and make sense of nutritional information and contribute to individualized treatment programs. Treatment programs can focus on a wide array of issues, including achieving optimal nutritional health, disease prevention and treatment of chronic diseases. The RDs are accountable to the College of Dietitians of Ontario.
Pharmacist
Pharmacists provide medication reviews and consultations to patients, their families and other healthcare professionals. Our Pharmacists perform individual patient assessment and medication histories, identify and rectify medication-related problems, provide education on medication use, collaborate with the FHT to improve medication management and the use of drugs for chronic diseases. Our Pharmacists are registered with the Ontario College of Pharmacists.
Social Worker
Our Social Workers help individuals and families achieve optimal psychosocial functioning within their own personal context. They provide guidance and psychotherapy to FHT patients struggling through difficult times. Our Social Workers use short-term approaches, including Cognitive Behavioural Therapy, Mindfulness, Solution-Focused Therapy, Narrative Therapy and others. FHT Social Workers are registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
Administration
Our healthcare professionals are supported by a team of qualified and welcoming administrative staff, as well as finance, payroll and HR personnel. Our administrative team manages the FHT's payroll and employee benefits program, support the formation and implementation of various FHT policies and procedures, ensure fiscal responsibility, assist in the recruitment of new staff and overall, provide daily coordination of FHT activities.
After months of hard work and determination, the 7th floor has officially opened.
Hosting 6 clinical offices and over 20 physicians and various Allied Health Professionals, the NYFHT is excited in the progress we have made to make primary care more readily accessible to North York.
