NYFHT Services

A Family Health Team is an approach to primary healthcare that brings together different healthcare providers to coordinate the highest possible quality care for you, the patient.

Family Health Teams are designed to support Family Physicians by providing complimentary healthcare professionals such as Registered Nurses, Nurse Practitioners, Registered Dietitians, Social Workers, Pharmacists and other healthcare providers.

The following healthcare providers work collaboratively with your Family Physician to provide you with the very best primary care possible:

Nursing Services

Our Registered Nurses are the gateway to your care. The RNs work closely with your physician and FHT Social Workers, Registered Dietitians and Pharmacists to navigate your healthcare needs, by providing clinical assessment and treatment, as well as directing you to the appropriate healthcare provider. Our RNs work closely with your Family Physician to:

  • Perform assessments and obtain medical histories
  • Administer injections and vaccinations
  • Deliver health education on an array of issues, such as blood
  • pressure, diabetes, smoking cessation, heart health, diet and cholesterol levels
  • Conduct EKG tests and venipunctures
  • Conduct cancer screening
  • Manage wound care and dressings
  • Respond to your questions over the telephone regarding infections, symptoms and test results

Similar to the RNs, our Nurse Practitioners also work with your Physician to enhance your primary healthcare experience. NPs are uniquely trained with advanced practice skills to meet the increasing demand for primary healthcare services. Our NPs work with your Physician to:

  • Perform a wide variety of assessments such as pap tests, breast exams, prenatal care, elder care and well baby visit check-ups
  • Deliver immunization and vaccination programs
  • Conduct physical examinations
  • Order and interpret diagnostic and laboratory tests
  • Diagnose and treat acute illnesses, chronic diseases, infections and injuries
  • Dispense and administer medications and treatments

Overall, our nursing services provide your Physician with increased clinical support so that your primary healthcare needs are met in a timely, efficient and comprehensive manner.

Dietitian Services

Our Registered Dietitians strive to improve your health through identifying nutrition problems and assessing your nutritional condition to develop healthcare plans and monitor the effectiveness of these plans. The NYFHT RDs offer individual and group counseling to educate you on a variety of nutrition topics.

Currently, our RDs offer the following groups:

Heart Health: This is a once-a-month introductory seminar for NYFHT patients with high cholesterol or high blood pressure. If you have been diagnosed with high cholesterol or high blood pressure, you must attend this seminar before a one-on-one consultation with a Registered Dietitian can be scheduled.

Healthy You: This is an 11-week group weight loss program open to any patient of the NYFHT interested in general weight loss with no present health conditions. This program is held 3 times a year.

Pre-natal: This is a general information session covering nutrition recommendations during pregnancy. It is held every other month and is open to any patient of the NYFHT who is either planning to become pregnant or currently pregnant.

You may wish to consult with one of our Registered Dietitians to:

  • Learn about nutrition recommendations during pregnancy
  • Help you manage existing health problems, which include but are not limited to, food allergies, hypertension, high cholesterol and Irritable Bowel Syndrome
  • Assist you to achieve a healthy weight
  • Discuss how to meet your daily nutritional requirements
  • Achieve optimal bone health
  • Plan healthier meals
  • Learn how to read food labels
  • Deal with picky eaters

If you wish to meet with one of our Registered Dietitians, please ask your Physician for a referral.

Pharmacy Services

Our Pharmacists are highly educated and trained professionals who work with your Physician to ensure the most appropriate use of medications to improve your health. They are able to work with you to address a broad range of issues, which may include:

  • Individual medication reviews to simplify medications, ensure compatibility, and determine appropriate dosing times
  • Educate you on how to manage or prevent side effects from your medications
  • Manage your medication list, which may be needed if you've had changes due to a recent hospital visit or a similar situation Our Pharmacists are available to answer any medication related questions you may have.

You can see one of our Pharmacists for one or more appointments as needed, and can be re-referred at any time. Please speak with your Physician if you wish to see one of our Pharmacists.

Social Work Services

Our Social Workers provide short term counseling, advocacy, and community referrals in regards to a wide array of issues such as anxiety, depression, or any psychosocial stresses. The Social Worker can:

  • Maximize your functioning and independence by providing psychotherapeutic counseling services (in individual, group or family formats)
  • Advocate on your behalf within the healthcare community to assist and facilitate further support
  • Provide you with information on community resources and facilitate access to these resources
