What is primary care?

The Ministry of Health and Long-Term Care defines primary care as "...the patient's first point of contact with a doctor or a health care team."

Primary care can include disease management and prevention (such as diabetes and other chronic diseases), disease cure, palliative care, health education and promotion and referrals to specialists.

What is a Family Health Team?

The Ministry of Health and Long-Term Care defines a Family Health Team as "...an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient. Designed to give doctors support from other complementary professionals, most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible."

How is the NYFHT specific to the North York area?

The NYFHT works in partnership with our Physicians, healthcare professionals, the Ministry of Health and Long-Term Care, Central LHIN and other community groups to identify areas of healthcare needs in the North York community and develop programs to address these needs.

What types of healthcare professionals work at the NYFHT?

Although the composition of Family Health Teams varies greatly depending on its community's needs, the NYFHT is able to provide you with Pharmacists, Registered Nurses, Nurse Practitioners, Registered Dietitians and Social Workers to improve your primary care experience. Please speak with your Physician if you are interested in obtaining the services of any of our healthcare professionals.
