Smoking Cessation Program

We are pleased to announce the launch of our new Smoking Cessation Program, starting Tuesday, October 11, 2011. Comprehensive one-on-one smoking cessation counselling and follow-ups will be offered to patients. Pharmacotherapy options will be discussed. Nicotine replacement therapy (NRT) products (patches, gums, lozenges, and inhalers), if indicated, can be provided at no cost to patients [4 weeks supply at a time, up to a maximum of 26 weeks in a year] as part of the STOP with FHTs study that is funded by the Ontario government in partnership with the Centre for Addiction and Mental Health (CAMH).

Initial assessments will generally take 45-60 minutes [patient will need to fill out consent form and baseline demographic information to participate in the STOP with FHTs study in order to receive NRT products at no cost]. Subsequent follow-up encounters will be approximately 15-30 minutes.

Diabetes Education Program

The NYFHT Diabetes Education Program is made up of a nurse and dietitian, both certified Diabetes Educators.

The Program currently offers both group classes and individual counselling for adults only.
